Your First Visit
You will need to fill out some forms prior to your first visit. To save time at your first visit, you may download and print forms here.
If you have insurance coverage, please also fill out the Assignment Of Benefits*.
Finally, please read our HIPAA Information Form. You will not need to print this document out. Please do fill out the HIPAA Acknowlegement Form*.
We want your child’s experience at the dentist to be as positive as possible. It is completely normal for children to have some anxiety about a new experience, and it is even normal for some children to cry because of fear of the unknown. Rest assured, even the most anxious children will become more and more comfortable as they have positive, painless experiences at the dentist.
Please don’t over-prepare your child for their first visit. Children can sense whether or not you are anxious about the appointment, and will behave accordingly.
The most important part of your first visit will be an oral exam with Dr. Callahan. The doctor will check for normal oral development and look for any signs of tooth decay or oral disease. We will give you some tips on preventative care. Decisions about x-rays, cleanings, fluoride treatments, etc. will be made based on your child’s age, level of anxiety, and dental needs. Parents will always be a part of the decision-making process.

Directions from I-25: Take the Woodmen Rd. exit and go EAST on Woodmen Rd. to Academy Blvd. Turn right (south) on Academy and go to Dublin Rd. Turn right on Dublin and immediately turn left on Wall Street. After turning onto Wall Street, you will see the Dublin Health Center on your left. We are on the lower level, so use the main entrance on the West side of the building.
